How to complete the form:

1. Provide name and email information (so musicians can respond). Include your phone number if you prefer to be contacted that way.

Note: Contact information is provided only to professionals participating in the RFI program - and only for the purposes of responding to your request. Your information is not shared with others or used for any other purpose.

2. Indicate your wedding date, location and time. This helps musicians know if they're available - to avoid wasting your time.

3. Indicate whether you are looking for music for your ceremony, reception or both.

4. Detail your specific information requests. Are you looking for a band to dance the night away at your reception? A harpist to add elegance and emotion to your ceremony? Both? Pianist and vocalist for cocktail entertainment? Something else altogether? The more information you include, the better the information you'll receive in response!
