How to complete the form:

1. Provide name and email information (so appropriate officiants can respond). Include your phone number if you prefer to be contacted that way.

Note: Contact information is provided only to sites participating in the RFI program - and only for the purposes of responding to your request. Your information is not shared with others or used for any other purpose.

2. Indicate your wedding date, location and time. This helps officiants determine whether they are available for your date - to avoid wasting your time.

3. Detail your specific information requests. Include religious/cultural concerns, budget questions, hours, etc. The more information you include, the better the information you'll receive in response!

Once you click the "send" button, your request for information will be routed by The Wedding & Special Event Yellow Pages to the matching officiants in our system.
